Abstract:

Introduction: Phyllodes tumors (PTs) are rare breast neoplasms with an incidence of 0.3% to 1% in relation to all primary breast tumors (1). With an Incidence rate of 2.1 per 1 million women (2). They are classified as either  benign, malignant and borderline according to the WHO. More rare presentations of these tumors include bilateral synchronous or asynchronous disease and/or unilateral multifocal disease. With the exact incidence rate being difficult to quantify due to their overall rarity. Surgery is the treatment of choice, but it may vary from an excisional biopsy to a total mastectomy(3). This case describes a 47year old female patient who presented with bilateral benign phyllodes tumor who underwent bilateral total mastectomy.

Case Report: A 47 year old female was initially seen due to a giant unilateral right breast mass. Patient underwent imaging and diagnostics which then noted breast mass at the right and multifocal breast masses at the left. Few days later bleeding was noted at the right breast and was urgently scheduled for total mastectomy. The pathology noted bilateral benign phyllodes tumors with no significant stromal atypia, overgrowth, increased mitosis (>4hpf) or overt invasion was noted. Based on a history of a rapid bleeding enlarging right breast mass and a multicentric left breast masses, bilateral total mastectomy was done. 

Conclusion: We present one of the rare cases in literature of a bilateral phyllodes tumor. This highlights the importance of proper diagnosis for optimal and adequate treatment of these rare cases.

Bryan Matthew H Miraflor is Filipino-American dual ciHzen. Born in Philadephia, Pennsylvania,USA. He spent his childhood and formaHve years half in San Diego California and half in Dumaguete City in the Philippines.He is a medical doctor who graduated from SillimanUniversity Medical School year 2018, did his post graduate internship at Silliman University Medical Center 2018-2019 and passed the Physician Licensure ExaminaHon in 2019. He is currently a 1st year general surgery resident at the Silliman University Medical Center.
